Vision Service Plan

Vision Service Plan has a method for delivering benefits. Now you can receive quick and direct access to vision care without completing a request card or calling for a benefit form.

Call a VSP participating doctor for any appointment and identify yourself as a VSP member. (If you need assistance locating a VSP doctor, call VSP at 1-800-877-7195) or through the internet http://www.vsp.com and you can access the providers off the web pages.
You will need to know your social security number.
After you've scheduled your appointment - your participating doctor will do the rest - verify coverage and bill VSP. YOU DO NOT NEED A CLAIM FORM.
