Identification & Recruitment Forms
The forms in this section provide staff with useful tools to help them identify migrant students within districts and the community e.g. flyers, interest forms etc.  In addition, there are forms to help document activities and tasks related to identification and recruitment.

MEES Forms
These forms are both Region 2 developed and State required forms for the MEES projects throughout the Region.  The Region 2 forms are identified with an ME number and help document the services that are provided to preschool age children.

Regular Year Forms
The forms in this section serve many purposes from documenting services to migrant students to providing accountability documentation.  These are also identified with an ME number to facilitate staff usage.  Some of the forms also have instructions included.  Many are optional and are placed to serve as tools.

Summer Forms
These forms will provide similar purposes as the regular year forms, but focus on the summer programs, as funding for summer is different than regular year funding.  Here also they are have an ME number and some are required while others are optional.

Parent Forms
These forms are related to parent activities from sign-in sheets to Parent Mentor stipend forms.  They also have ME number’s to help identify with a number rather than a title.  As with the others, some are required and some were developed as tools and are optional.  Many of the forms will be useful for the District Parent Education Liaison.

State Migrant Education  Forms
The forms in this section have been developed by the State Department of Education including all of the service agreements forms needed each year of the five-year cycle.
